New and returning scouts must complete Medical Form Parts A and B. New scouts must also complete the Youth Application form and the Pack 97 Info Form. Click the links below to access the forms.
For the 2011-2012 scout year, dues are $63. Add $12 if your scout would like to subscribe to Boy’s Life Magazine. Make checks payable to "Pack 97." Please turn in all forms to your scout's Den Leader. Or you may also give the forms to Timothy McCole, Committee Chairman, Philip Lamsens, Treasurer, or Gene Lamis, Cub Master.
Youth Application. Turn in completed form to your scout's Den Leader or the Pack's Cub Master, Committee Chairman, or Treasurer. A scout need apply only once, upon initial entry in to the Pack.
Medical Forms A & B. Turn in completed Forms A & B each scout year.
Pack Registration Form. Turn in completed Registration Form each scout year. |
